Please note that only the Chicken Barbeque form (pg 8), Borrowing classes (pg 14-16) and 4-H Club Vendor Booth application (pg 83) are included in this download. All other 4-H exhibitor registration forms must be obtained separately under the Exhibitor Registration and Horse Show Registration links below.
Exhibitor Registration -- All the necessary documents and information for registering as an exhibitor. Horse Show information and registration forms are listed below
Registration Forms can be filled out on-line and printed. Registration Forms must be returned by July 29th to the Tolland County Extension Center, 24 Hyde Avenue, Vernon, CT 06066. Horse Show Registration is due July 22, 2011 to avoid post entry fee.
1. Every exhibitor must fill out the General Exhibitor registration form including The 4-H Code of Conduct. Mail it with $1.00 to cover cost of the accident insurance policy for each member during the fair. This fee is included in the Horse Show Registration fee.
2. Every 4-H member that will be on the grounds at any time without a parent present must file a health form with the 4-H Fair office. Helath forms are available at the Tolland County Extension Center or on the 4-H website http://www.4-h.uconn.edu under forms. It is each parent's responsibility to file this health form with the fair office or provide it to the 4-H leader taking responsibility for your child.
3. 4-H'ers entering animals must fill out and return all the appropriate entery form(s) for each species. Use a different form for each species. Livestock permanent ID is required for all animals. ID must match the number on the health form. Any livestock without this will not be allowed on fairgrounds.
Horse Show Registration -- Horse Show information and registration is found here. All 4-H members must include a General Exhibitor Form including the 4-H Code of Conduct in addition to their show registration form. Horse Show Registration is due July 22, 2011 to avoid post entry fee.
